Join us for a dress rehearsal open shoot of NUTCRACKER FRIDAY December 3, 2021, at PIKE PERFORMING ARTS CENTER.
We will not have our regular club meeting on December 2, 2021
Note that if you wish to photograph Gregory Hancock Dance Theatre dress rehearsals, you must download, print, fill out and bring with you this COVID Visitor Affidavit.
In addition, the access and screening process will be as follows:
1. A mask or other facial covering is required and must be worn over the mouth and nose at all times while in the building.
2. Complete the Visitor Affidavit and Release of Liability. (Photo Venture members are asked to complete their forms before arriving to the Gregory Hancock rehearsals.)
3. Hand the completed form to the security guard and submit to a temperature screen.
a. If temperature is above 100° F, the visitor will be asked to wait 10 minutes and submit to another screen.
b. If temperature is still above 100° F, the visitor will not be admitted further into the building and will be asked to leave.
4. After Affidavit and Temperature Screen are completed successfully, the Visitor will be issued a stick-on badge to be worn while on premises.
